Scheduling Your Appointment
We understand your time is invaluable. To ensure adequate time is scheduled for your appointment, please be prepared to provide the following information to the receptionist:

Once Your Appointment Is Scheduled
Please bring the following items with you to every visit:

Driver's license or other valid government-issued ID

Insurance card and any co-payment due for the visit

Referral forms (if applicable)

X-rays and any related studies (if applicable)

Any necessary new patient or follow-up forms

Your Guide to Patient Cost Share
We know that insurance can be difficult to understand. Here are some simple explanations of common terms you may find:

Annual Limit: the maximum dollar amount an insurance company will pay for claims within a given year
Example - Medicare will pay $2,010 per year for physical therapy visits

Co-insurance: the percentage of costs of a covered healthcare service you pay after you've paid your deductible
Example - After paying my deductible, my insurance pays for 80% of my visit, so my co-insurance is 20%

Co-payment (co-pay): the pre-set charge that an insurance plan requires its members to pay for each visit to a healthcare provider
Example - My co-pay is $25 for doctor visits and $50 for specialty visits

Deductible: the dollar amount an insurance company requires its members to pay every year before insurance benefits kick in
Example - After I pay my $2,000 deductible, my insurance kicks in and I have a co-pay for covered services.

Out-of-pocket max: the most you have to pay for covered services in a plan year
Example - After I pay my $4,000 out-of-pocket max with my deductible and copays, my health plan pays 100% of the costs of covered benefits for the rest of the year.
